Dress Code
Order Your Uniform
Families can purchase uniforms through Land's End (preferred school number is 900104894). All shirts, sweaters, and fleece jackets must have the official school logo on them.
Please refer to the Land's End School Uniform Fit Guide for appropriate sizing.
General Guidelines
General Statement: The purpose of the Student Uniform, Dress and Grooming Code is to foster a respectful, professional learning environment with particular emphasis on neatness, modesty, and cost effectiveness.
Parents or guardians are expected to support the school’s enforcement of the Student Uniform, Dress and Grooming Code, thus reinforcing an environment best suited to learning and faith development. When in doubt, ask for clarification from the administration at least one month prior to coming to school with questionable dress or grooming.
Our purpose in establishing a uniform policy is threefold:
- The school uniform helps distinguish our students.
- The school uniform reflects our unity of purpose.
- The school uniform reflects our equality before the Lord, reminding us that we work as members of the Body of Christ, rather than for our own glory.
A uniform policy produces many advantages, including moral and spiritual:
- A uniform policy encourages modesty, neatness, and humility, not drawing attention to the individual. (1 Peter 3:3-4)
- A uniform policy recognizes the distinct differences between male and female students. Students must dress according to their biological sex. (Deuteronomy 22:5)
- A uniform policy cultivates an atmosphere of order. Thus the classroom becomes peaceful, polite and focused. One’s appearance is frequently an indicator of one’s attitude toward doing well.
- A uniform policy removes temptations to judge others based on externals, to become too focused on fashion trends, or to be covetous of clothing of classmates. (Matthew 6:25)
- A uniform policy is cost-efficient. Our uniforms are of excellent quality and reasonably priced, so many parents find uniforms to be more cost-effective than maintaining wardrobes of “street clothes” for their students.
General Guidelines
- Clothing must be neat, clean and free of rips/tears/fraying.
- Clothing must fit properly and not be excessively tight or excessively baggy.
- Underclothing, with the exception of short sleeve t-shirts and solid white long-sleeved t-shirts, may not be exposed or showing
- No outerwear, except an NDA fleece jacket or NDA uniform sweater, may be worn or carried during the school day.
- Hats and sunglasses are to be removed upon entering the school building.
Pants
- Solid navy blue or solid tan khaki (cotton broad cloth/Chino) dress style pants. Land’s End has NDA-approved pants for purchase. Pants, regardless of where they are purchased, must be of comparable style. The NDA-website has pictures of acceptable pant styles.
- Solid navy blue or solid tan khaki shorts (cotton broad cloth/Chino style) may be worn from April through October. Shorts can be no shorter than 4 inches above the knee. Cargo and jogger shorts are not allowed. Land’s End has NDA-approved shorts for purchase.
- Cargo pants, jogger pants, denim pants, corduroy pants, cuffed pants, excessively tight pants, such as jeggings, leggings, skinny pants and spandex are not permitted.
Skirts
- Solid navy blue or solid tan khaki (cotton broad cloth/Chino) style skirt. Skirts must be worn to the knee and may not be excessively tight.
Shoes & Socks/Hosiery
- Shoes and socks/hosiery must be worn at all times. Shoes must have a closed toe. Flip- flops, sandals, soft-sole moccasins, slippers, clogs and Heely shoes are not permitted. Socks must cover both heels and toes.
- Solid colored nylons, tights & socks without texture or pattern are acceptable.
All items listed below are to be purchased through “Land’s End” by accessing the links on the NDA website. All items must have the official school logo on them.
School Uniform Shirts
- Solid navy blue, solid white or solid forest green polo (long or short sleeve)
- Solid light blue or solid white oxford (long or short sleeve); only the top two buttons may be unbuttoned
Sweaters
- Solid navy-blue zipper or button-down sweater (must be worn with school uniform shirt)

Spirit Days
The school administration may deem certain days as Spirit Days. Students should check the online
school calendar for designated Spirit Days. On Spirit Days, official NDA apparel may be worn in place
of school uniform shirts. All other attire must fall within the dress and grooming code. Official NDA
apparel refers to shirts purchased through the NDA Book Store or an NDA Club/Team.
Grooming
- All students’ hairstyles must be kept neat, clean, professional, and a natural hair color. Extreme hairstyles are prohibited. The administration has the right to determine what styles are acceptable.
- Boys must be cleanly shaven. Sideburns cannot extend below the bottom of the ear.
- A boy’s hair can be no longer than collar-length in back, not below the ears on the sides and not below the eyebrow.
- Face piercing of any kind, other than ear piercing, is not permitted. Boys may wear only one earring per ear that is a simple stud. Girls’ earrings may not hang below the chin.
- Visible tattoos are not permitted.
Dress Code Violations: Parents and students are expected to abide by this Code and be motivated by the spirit of the Code. When in doubt, ask for clarification from the administration at least one month prior to coming to school with questionable dress or grooming. The school reserves the right to interpret and take disciplinary action on violations of the Uniform, Dress and Grooming Code. In individual circumstances, the school reserves the right to declare certain clothing or grooming acceptable or unacceptable. A student is subject to disciplinary action for any dress that violates the specific requirements of the Uniform, Dress and Grooming Code or the spirit of it as well. Violations are referred to and interpreted by the administration, whose decision will be final.
Bring Your Own Device (BYOD) Policy

BYOD
- Any brand of Chromebook, Windows laptop or Macbook is allowed for daily use in the classroom or library at the discretion of the teacher.
- All BYOD electronic devices are prohibited from use during the school day in common areas (lunch, passing period between classes, bathrooms, assemblies, prayer services, masses, locker rooms, chapel, etc.). BYOD devices are prohibited from use in the locker rooms and bathrooms at all times. Suspicious use or failing to comply with the “Guidelines for Acceptable Use of BYOD Electronic Devices in Classrooms/Library” will be considered a violation of this policy. Students who use their BYOD electronic devices are subject to the guidelines and consequences as listed in the “Technology Acceptable Use Policy” and other school policies (i.e. Harassment/Bullying, etc.).
- Laptops are required to have antivirus software installed. Students are expected to ensure their device is fully charged before coming to school. Charging stations in common areas are on a first come first serve basis. The NDA library will have devices available for check out on a limited basis.
- Students are required to use the school WiFi at all times during the school day.
- Students who check out a school owned device will be responsible for that device. Parents/guardians and students sign an agreement form prior to long term check out. Students are responsible for transporting, storing and securing all devices. Notre Dame Academy employees are not liable for any device stolen or damaged on campus or at school activities.
Guidelines for Acceptable Use of BYOD Electronic Devices in Classrooms/Library
- BYOD electronic devices may be used for academic reasons at the discretion of the teacher.
- BYOD devices are to be off, closed or in storage until their use is directed by the teacher. (i.e. carrying case, backpack, etc.)
- The device must be visible to the teacher at all times when in use.
- The device must not disrupt or distract the learning process (i.e. make noise, etc.).
- The use of earbuds, headphones, etc. is at the discretion of the teacher.
- The charging of devices in the classroom/library is at the discretion of the teacher. Extension cords are not allowed.
- All printing on school owned printers will be through school owned devices. BYOD devices will not be able to print on school owned printers.
- Before leaving the classroom, all BYOD devices need to be closed and put back into storage (i.e. carrying case, backpack, etc.)
Consequences for Misuse
- First Incident: Two detentions
- Second Incident: Two detentions and handwritten behavioral reflection plan for improvement

Student Medicine Policy Forms
Student Medicine Policy
Students are not allowed to possess or take prescription or non-prescription medicine on campus or during school activities unless the parent/guardian has informed the main office or coach/moderator (in the event of an extracurricular activity) in writing. Failure to comply could result in a Code of Conduct violation. All prescription and non-prescription medicine will be administered to students in accordance with NDA’s policy related to Wisconsin Statute Sections 118.29, 118.291, and 118.292.

Medicine Administration to Students
Medications are given to students in the school setting to continue or maintain a medical therapy which promotes health, prevents disease, relieves symptoms of illness or aids in diagnosis.
Notre Dame de la Baie Academy shall administer medication in accordance with Wisconsin Statute Sections 118.29 and 118.291. The School may administer any prescription medication to a student in compliance with the written instruction of a practitioner and written consent from the student’s parent or guardian as defined by Wisconsin Statute Section 118.29. Administration of nonprescription medication requires the written instruction and consent of the student’s parent or guardian. Substances, which are not FDA approved (i.e. natural products, food supplements), will require the written instruction of a practitioner and written consent from the student’s parent or guardian. Students with asthma may possess and self administer a metered dose or dry powder inhaler with the written approval of the student’s physician and parent or guardian. Under Wisconsin Statute 118.292, students who may need epinephrine or glucagon in an emergency situation may possess an epinephrine or glucagon injector with the written approval of the student’s physician and written approval of the student’s parent or guardian if the student is a minor.
Medication administration may be delegated by a school administrator to any school employee or volunteer with proper training, supervision, and evaluation. Under the statutes, any school employee or volunteer who is authorized to administer medication is immune from civil liability for his or her acts or omissions in administering medication to a student unless the act or omission constitutes a high degree of negligence. Also under the statutes, any school administrator or principal who authorizes an employee or volunteer to administer a drug or prescription drug to a student will be immune from civil liability for the action authorized, unless a court determines that the action constitutes a high degree of negligence as defined in Section 118.29.
No school employee, except a health care professional, may be required to administer medication to a student by any means other than oral ingestion.
Procedures for obtaining and filing written instructions and consents for medication administration, and the protocols for storage, administration, and documentation are delineated in this policy’s Administrative Rule.
It is the responsibility of the student to take his or her medication at the assigned time from the assigned school official.
A parent/guardian may come to school or the school activity to administer prescription and non-prescription medicine to his or her student without completing the appropriate documentation with the school. In such a case, the parent/guardian assumes full responsibility for any consequences or effects on the student arising out of the administration or delivery of medication or of medical treatment of the student, and the parent/guardian agrees to protect, indemnify, and save the school harmless from any liability, including any costs or expenses arising out of any claim against the school under such circumstances.

Procedures: Medication Administration to Students
Training of Designee
The school administrator has the authority to delegate medication administration to a school employee or volunteer if the following are met:
- The employee or volunteer is administering medicine by oral ingestion only.
- The employee or volunteer completes any additional training as required by the school.
- School Personnel will be informed on a need to know basis when a student is taking medication for serious or chronic health conditions, so that they can observe the student for side effects to the medications.
Consent to Administer
Prescription Medications
A written, signed statement from the parent/guardian and a written, signed instruction from a practitioner must be on file at the school authorizing school personnel to administer any medication (“Parent(s)/Guardian Medication Authorization Form” & “Medical Provider Authorization Form”). The statement must include:
- Medication name, dose, route frequency, time/conditions, duration
- Reason for medication
- Precautions, possible untoward reactions, and/or interventions
- Name of practitioner
- Parent/guardian signature, practitioner signature, date
- Student name and date of birth
Requests must be renewed each year or more often if changes in dosage occur. All changes will be noted on the medication administration record, dated, and initialed by the person assigned by the Principal to be responsible for maintaining that record. Prescription medications must be supplied in the original pharmacy-labeled package indicating the name of the student, the name of the prescriber, the name of the prescription drug, the correct dosage, the effective date, and administration instructions. Parents/guardians may come to school to administer prescription medication.
Non-Prescription Medications
Non-prescription medication (over-the-counter) which is FDA approved can be administered. A written, signed statement from the parent/guardian must be on file at school authorizing school personnel to administer such drug products (“Parent(s)/Guardian Medication Authorization Form”). The school may supply to the student drug products listed on the form and/or equivalent generics. Non-prescription drugs must come to school in the original manufacturer’s packaging with ingredients and recommended therapeutic dose. Non-prescription medications must be supplied in the original container with the student’s name affixed. Any non-prescription medication intended for long-term use on a daily basis or to be used in a dosage other than the recommended therapeutic dose must be accompanied by a practitioner’s signature approving of such dosage. Parents/guardians may come to school to administer non-prescription medication.
Food Supplements and Natural Products
For the safety and protection of students, food supplements and natural products will not be given in the school setting unless approved by the FDA or prescribed by a practitioner. The following criteria must be met:
- An original container is provided.
- Appropriate dosing for student is clearly stated on the label/packaging insert.
- Possible untoward effects are listed.
- Signed “Parent(s)/Guardian Medication Authorization Form”.
- Signed practitioner consent (“Medical Provider Authorization Form”) if non-FDA approved.
- Use for student is indicated.
- Parents/guardians may come to school to administer food supplements or natural products.
Emergency/Self-Administered Medication
Students with asthma may possess and self administer metered dose inhalers or dry powder inhalers for the purpose of preventing or alleviating the onset of asthmatic symptoms. The student must have the written approval of the student’s physician and the written approval of the student’s parent or guardian (see “Asthma Inhaler Administration Authorization” form). A copy of this approval must be filed with the main office at the school, and shall be maintained in the medical record. The medical record is to be updated annually.
Any other self-administered medication (i.e. insulin pumps, epinephrine pens, etc.) must have the appropriate authorization form (“Emergency/Self-Administered Medication Authorization” form) and written consent of an administrator on file. Requests must be renewed each year or more often if changes in dosage occur.
Medication Storage
Medication will be stored in a secure location. Medication which needs to be accessible to the student will be stored in an appropriate location per student need (i.e., emergency medications). Medication will be stored to maintain quality (i.e., refrigeration). The parent/guardian shall pick up unused portions of medication within seven days after the completion of the school year or when medications have been discontinued. After written/verbal notification, medications will be destroyed.
Documentation
An accurate individual student record of administered medication will include:
- Demographic data such as name, birthdate, level/grade, school year.
- Medication name, dose, date/time given.
- Signature of person administering.
- Dose changes, dated, with the signature of designee.
Documentation of medication must be completed immediately after the administration to the student to assure accuracy and safety. Documentation of medication errors must be completed immediately. Parent/guardian will be notified via phone of any errors.
The Student Medication Record(s) will be maintained in the student’s school medical record after discontinuation of the medication.
Rights and Responsibilities
Students have the responsibility to:
- Follow the school medication policy.
- Take the initiative to go to the designated school person who is to administer the medication at the designated time.
Parents have the responsibility to:
- Familiarize themselves with this student medication policy.
- Follow the school medication policy.
- Inform the school (main office, coach, extracurricular moderator, etc.) if their student needs to take prescription or non-prescription medicine.
- Sign and deliver to the school all required authorization / consent / instruction documents required under this policy.
Designated school personnel have the responsibility to:
- Administer the medication as per the guidelines on the appropriate forms.
- Maintain the medication administered at school in a secure place which also maintains medication quality (i.e., refrigeration for liquid antibiotics).
- Report to the parents any dose changes, inconsistencies, or medication side effects which are observed or reported.
- Keep a copy of the Medication Policy in an accessible spot for immediate reference.
- Document all medication administered or reason medication may not be administered (absent, refusal).
- Report errors and/or missed medications to the student’s parent/guardian immediately.
- Note: Designated school personnel have the right to refuse to administer medication to students when the medication administration procedures as described in Section II above have not been completed.
School administration has the responsibility to:
- Use professional judgment in carrying out the policy.
- Provide training, supervision, and evaluation of the administration of medication in the school.
- Maintain records of staff completion of medication administration training.
Distribution of Policy and Liability Waiver
Any parent/guardian of a student who needs to possess or take prescription or nonprescription medicine at school or at school activities shall complete, sign, and deliver written directions to the school on forms provided by the school.
All school employees or volunteers who are authorized to administer drugs to a student shall receive a copy of this policy and shall be advised that, pursuant to Wisconsin Statute Section 118.29, they are immune from civil liability for any acts or omissions in administering a prescription or non-prescription drug to a student in accordance with this policy unless the act or omission is found by a court to constitute a high degree of negligence as defined in the statute.
Moreover, school employees, volunteers, and administrators are also protected with civil liability immunities, under the statutes, if they prohibit or allow a student to use or to not use an inhaler if the employee has a good faith belief that the student uses the inhaler before exercise to prevent asthmatic symptoms or at any time to alleviate asthmatic symptoms, that the student has the written approval of a physician and of his or her parents, and that the student has provided the school principal with a copy of the approvals. Other civil liability exemptions are also available, under the statutes to any person who renders emergency care at the scene of an emergency, and acts in good faith, and to any school employee, officer, or volunteer, who in good faith attempts to prevent suicide by a student.
The school administrator who authorizes an employee or volunteer to administer a drug or prescription drug to a student is immune from civil liability for the act of authorization unless it constitutes a high degree of negligence as defined in Statute Section 118.29.

Personal Electronic Devices
Cellular phones, digital cameras, iPods/mp3 players, and all other personal electronic devices must be turned off and not used during the school day, unless it is for academic reasons and the classroom teacher has given permission for its use. Smart watches and fitness tracker devices are prohibited during the school day. All personal electronic devices are prohibited from use during the school day in common areas (lunch, hallways, bathrooms, assemblies, prayer services, masses, locker rooms, passing period between classes, etc.). Personal electronic devices are prohibited from use in the locker rooms and bathrooms at all times. Suspicious use or failing to comply with the “Guidelines for Acceptable Use of Personal Electronic Devices in Classrooms/Library” will be considered a violation of this policy. Students who use their personal electronic devices are subject to the guidelines and consequences as listed in the “Technology Acceptable Use Policy” and other school policies (i.e. Harassment/Bullying, etc.). Students are responsible for transporting, storing and securing all devices. Students may use cell phones in the Main Office during the school day with permission. Notre Dame Academy, its staff and employees are not liable for any device stolen or damaged on campus or at school activities.
Guidelines for Acceptable Use of Personal Electronic Devices in Classrooms/Library
- Personal electronic devices may be used for academic reasons at the discretion of the teacher.
- Students must ask the teacher for permission before taking the device out of storage (i.e.carrying case, backpack, pocket, etc.).
- The device must be visible to the teacher at all times when in use.
- The device must not disrupt or distract the learning process (i.e. make noise, etc.).
- The use of earbuds, headphones, etc. is at the discretion of the teacher.
- Personal electronic devices may not be used to listen to music.
- Before leaving the classroom, all devices need to be turned off and put back into storage (i.e.carrying case, backpack, pocket, etc.).
Consequences for Violation
- First Incident: Two detentions and device surrendered to Main Office where it may be picked up after school
- Second Incident: Two detentions and device surrendered to Main Office where it must be picked up by a parent/guardian and Loss of Privileges
- Third Incident: 1 - 3 day suspension
- Fourth Incident: Recommendation for expulsion

Student Use of Technology
Notre Dame Academy supports the use of technology resources for the sole purpose of achieving NDA educational goals, standards and curricular objectives. In addition, technology provides efficient and effective means to teach collaboration and organizational skills necessary for the 21st century. Technology can be a valuable tool for school related activities, events, etc. Students are expected to be ethical Digital Citizens in their use of technology and to report inappropriate and unsafe use to school officials. NDA will make every reasonable effort to utilize security software that will filter access to information that is not suitable for minors.
Acceptable Use Code of Conduct for Technology
Student access is a privilege, not a right. Whether students are using their own or NDA-owned devices, students are responsible for demonstrating appropriate behavior while using technology. Users should not expect stored files to be private. At any time, the student’s parents, NDA faculty and/or staff may request access to the student’s files for any reason.
Students are responsible for their own behaviors and are expected to comply with the following Code of Conduct for Technology:
- Whether personally or NDA owned students are responsible for physical damage caused to devices by food, drink, accidental damage or intentional misuse.
- The Internet is to be used for curriculum objectives.
- Students should never share personal information, including phone number, address, social security number, birthday, or financial information over the Internet without adult permission. Students should recognize that communicating over the Internet brings anonymity and associated risks, and should carefully safeguard the personal information of themselves and others.
- Network security interruptions, such as introduction of viruses or deletion of files are not permitted.
- Printers may be used for educational school related documents only. At school, printing must be done with NDA-owned devices.
- Offensive messages or pictures may not be sent, displayed or downloaded.
- Users of technology resources should respect the rights and privacy of others. Each student should only use their own assigned password/ID and may not trespass into the files, folders or work of other users. A student should not allow other students to access the computers, networks or Google Drive using his/her password/ID.
- NDA will provide an email address and related Google Drive for students to facilitate communication and collaboration. In addition, the NDA issued email address provides a safe, simple and reliable way to verify other users (teachers and students). Students will be expected to use their NDA issued email address and Google Drive for all academic and school related use (activities, calendar, Haiku, etc.). Students are expected to check their NDA issued email address on a regular basis, especially on school days. Students’ NDA issued email and Google Drive are maintained for one year after graduation and then are deleted.
- To ensure the safe use of the internet during the school day, students will be required to login and use the school WiFi at all times. The school filter is designed to create a safe environment for our students to learn. If the filter blocks a student from a website, they may seek permission from the classroom teacher or Director of Technology for access. The school will provide sufficient bandwidth to ensure that academic learning is maintained throughout the school day.
- Copyright laws should be respected. All quotations, references, graphics and sources must be cited to give credit to the originator as stated in the NDA Honor Code.
- School issued devices are the property of Notre Dame Academy. Any misuse of these devices, including taking pictures or videos, leaving notes or non-educational documents, updating the iOS/OS, changing the background, icon setup or wallpaper may result in disciplinary action.
- Taking pictures or making or transmitting any video or audio recording of other students or school staff is prohibited at all times, unless the student has obtained advance permission from a teacher or administrator.
Violations of established policies and procedures will result in a disciplinary referral with the consequences as listed in the Handbook. Individual users are ultimately responsible for their activity when using technology. NDA may review files and communications to ensure the safety of students, maintain system integrity and ensure that users are demonstrating acceptable use of technology. NDA will not be held liable for any information that may become lost, damaged or unavailable due to technical or other difficulties. NDA is not liable for losses, claims or demands against NDA or any user by any other party based on the user’s unethical or illegal use of technology.
Notre Dame Academy reserves the right to impose consequences for inappropriate behavior that takes place off campus and outside of school hours. Thus, inappropriate use of technology (for example, on a home computer or personal electronic device), may subject the student to the consequences of the Technology Acceptable Use Policy as well as other pertinent sections of the Handbook. Inappropriate use includes, but is not limited to: harassment; use of school name or logo; remarks directed to or about other students, teachers, staff or administrators; offensive communications and safety threats.
Notre Dame Academy does its best to educate students about the appropriate use of technology at all times. NDA monitors student use of technology to the extent possible when students are at school or involved in school related projects (email, Google Drive, Haiku, internet blogs, social networking sites, chat rooms, etc.). Like other policies in the Handbook, students represent NDA, its mission, and Core Values at all times through their conduct using technology, internet, social media, etc.
While our filtering system establishes parameters for appropriate use within our building, students and parents/guardians are primarily responsible for the appropriate and ethical use of technology, especially in the home and when devices use cellular data plans. Parents/guardians are responsible for the filtering of devices at home or when cellular data plans are in use.
Technology Violations and Consequences Policy
The following consequences will be applied to misuse of any Notre Dame Academy’s Technology Equipment:
Misuse of NDA-owned Computers and Devices
- NDA-owned equipment is to be used according to the guidelines set forth by the Director of Technology, classroom teacher and the Technology Acceptable Use Policy. Examples of misuse include, but are not limited to: nonacademic use, viewing or downloading inappropriate material, etc. The severity of the consequence will depend on the extent of the misconduct.
- First Incident: 2 detentions

Misuse of School Resources
- NDA-owned computer, printers and devices are shared by the entire school community. As a school community, we must steward our resources appropriately. Violations of this policy include, but are not limited to: printing of inappropriate documents/pictures, wasting of resources by printing non-educational items, excessive printing, etc.
- First Incident: Loss of Privileges for NDA-owned equipment for 2 weeks
- Second Incident: Loss of Privileges for NDA-owned equipment for 4 weeks
- Third Incident: Loss of Privileges for NDA-owned equipment for 1 semester
Bypassing Internet Filter
- Students must not use websites that attempt to bypass NDA’s Internet Filter to access blocked websites or use other methods to bypass NDA’s Internet Filter. The severity of the consequence will depend on the extent of the misconduct.
- First Incident: 2 detentions
- Second Incident: 1 - 3 day suspension and Loss of Privileges
- Third Incident: Discipline Committee Meeting and Recommendation for Expulsion
Damaging Equipment
- NDA-owned technology resources and equipment should be handled with care. Damage to NDA-owned equipment whether intentionally or unintentionally caused by food, drink or misuse will result in the student paying for the replacement costs for the damage at current pricing levels in addition to the consequences below.
- First Incident: Loss of Privileges for NDA-owned equipment for 3 weeks
- Second Incident: Loss of Privileges for NDA-owned equipment for one semester
- Third Incident: Privileges for NDA-owned equipment revoked
Inappropriate Communication
- Technological communication will follow the Notre Dame de la Baie Academy “Guidelines for Technological Communication.” Inappropriate communication includes misuse of school email, sending inappropriate material, etc. The severity of the consequence will depend on the extent of the Misconduct including being considered Harassment/Bullying.
- First Incident: 2 detentions
- Second Incident: 1 - 3 day suspension and handwritten behavioral reflection and plan for improvement
- Third Incident: Recommendation for expulsion
Privacy of Logon Information
- Users of technology resources must respect the rights and privacy of others. Each student may only use his/her assigned password/ID and may not trespass into the files, folders or work of other users.A student should not allow other students to access the computers or networks using his/her password/ID. If the student feels his/her credentials have been stolen, the student should report this to a school official immediately. The credentials will be changed as soon as possible by the Director of Technology. The severity of the consequence will depend on the extent of the misconduct.
- First Incident: 2 detentions
- Second Incident: 13 day suspension and Loss of Privileges
- Third Incident: Recommendation for expulsion
Network Security
- Students should not tamper with network security. This includes introduction of viruses, attempts to hack into the network or intentional access of network servers, programs or deletion of files.
- First Incident: Discipline Committee Meeting and Loss of Privileges, possible suspension or expulsion

- Copyright laws must be respected. All quotations, references, graphics and sources will be cited to give credit to the originator. Consequences will follow the NDA Honor Code

NDA Electronic Surveillance Use Policy
Notre Dame Academy uses electronic surveillance technology inside the building and on campus property for the purpose of maintaining a safe and orderly educational environment, for identifying disciplinary issues, for minimizing theft, vandalism, criminal activity, bullying, harassment, and for enforcing school policies and rules.
Parent(s)/guardian(s) are notified annually via the Parent & Student Handbook that surveillance technology is used inside the building and on campus property. This policy will be referenced in employee handbooks and on the school website.
Surveillance technology will be used according to the following guidelines:
D. Only the school Administration and/or his or her designee shall be authorized to view the recording for the purpose of documenting a concern and determining student involvement. The surveillance technology recording shall not be available for viewing by the public in general, employees in general or media. For the purposes of viewing footage, the President’s designees shall be:
1. Principal, Associate Principal, Main Office staff, Director of Technology and/or Director of Maintenance.
E. Pursuant to state and federal law, the school reserves the right to provide copies of recordings to law enforcement agencies as deemed appropriate by the President and/or his or her designee.
F. The surveillance system may be used as a resource for investigations. Should a recording become part of a student disciplinary matter, the digitally segregated recording will become part of the student's behavioral record consistent with the school’s records policy and procedures. The school reserves the right to introduce surveillance technology recordings at any disciplinary hearing involving student misconduct or rule violations.
1. Parent(s)/guardian(s) of students may be allowed to view the recording along with a school administrator or authorized school staff member. In limited circumstances, students may be allowed to view a recording where appropriate.
2. If more than one student is identifiable in a given frame or a series of frames, neither the student to be disciplined nor the student's parent(s)/guardian(s) will be able to view the recording unless:
a. The recording can be edited or altered so as to render all other students unrecognizable; or
b. Written consents are obtained from the parent(s)/guardian(s) of the other students. Consents must be signed, dated and must specify the records to be disclosed, the purpose of the disclosure, and the party or the parties to whom disclosure may be made. All written consents will be facilitated by the school using official school forms.
H. Except as provided for herein, surveillance recordings shall generally be available for no longer than 30 days and may be written over by subsequent recording. The school administration may extract and retain footage for any legitimate reason it deems necessary. In the case of footage retained as part of a student behavioral record, the footage shall be retained consistent with the school’s student record retention policy. All other saved recordings shall be disposed of in a secure manner at such time as the reason the footage was retained no longer necessitates its retention
Except as required by court order or in the case of a student record transferred to another school of attendance, no person outside the school shall be provided with a copy of any retained surveillance footage. If any part of this policy is held to be invalid by law or future court action, that part shall be immediately altered to reflect such decision and operating procedure shall also reflect such change.

School Dance Guidelines
School Dance Guidelines
These guidelines emerge from Notre Dame Academy’s mission to promote Catholic Christian values that recognize the dignity of all people. Student behavior and attire at dances should reflect that mission. At NDA, dances are an opportunity for students to socialize in a supervised and safe environment and foster community.
General Regulations
All students are required to show identification for entrance. Students must enter by a specified time and may not re-enter after leaving a dance. Whether on-campus or off-campus, all school policies are in effect at dances.
Chaperones
NDA faculty, staff, administrators and parents will serve as chaperones for dances. At least 5 chaperones will be on duty at all times. Chaperones will closely monitor entrances and exits as well as restrooms and common areas. A breathalyzer will be available for use by trained school personnel if a student is suspected of being intoxicated.
Guests
Non-NDA students are permitted to attend some school dances. All non-NDA students must complete a guest pass which includes a signature from the parent of the NDA student as well as the parent of the guest. Forged signatures on a guest pass will result in forfeiture of attendance at current and/or future dances, no refund, and possible disciplinary action. All guests must show identification for entrance. Parents are welcome to visit all dances. Parents are asked to enter through the designated entrance and, for safety reasons, will also need to show identification for entrance.
Attire
Student clothing should be consistent with the mission and core values of Notre Dame Academy. Students are expected to observe standards of modesty and good taste. Low cut blouses/dresses are not permitted. Dresses/skirts may not be shorter than 4 inches above the knee. Any interpretation and judgment in these matters lies with the school personnel supervising the dance.
Music
All music should be consistent with the values and teachings of Notre Dame. Music must not contain lyrics, themes or innuendo that are violent, lewd, profane or otherwise offensive. Edited versions of offensive songs may not be acceptable if the underlying message is still present in the song. Music should encourage appropriate interaction among students. Music should reflect a variety of styles and genres. DJs will be provided these guidelines as well as instructions about appropriate music.
All dance styles must comply with standards of modesty and safety. Dancing that is unsafe, overly amorous or sexually suggestive is prohibited. Chaperones in attendance will be the final judges of the appropriateness of dance styles. Students must comply will the instructions of chaperones. Failure to do so will result in students being asked to leave and may result in additional disciplinary action. During the fall of 2008, the Notre Dame Academy Administration, Student Government moderators and the Student Senate have used a collaborative process to clarify dance styles and developed the following additional guidelines that have been approved by the Curriculum and Student Life Committee of the Board of Education:
- Sexually suggestive dancing will not be tolerated (known commonly as grinding). However, it should be noted, that any dancing moves or behavior deemed improper and/or indecent will not be permitted.
- A student will receive one warning for inappropriate dancing. If the behavior persists, the student will be asked to leave the dance. Students who dance inappropriately or break any of the school rules will be removed from the dance by one of the administrators, and the parents will be called and told the student has been asked to leave.
The dance will be stopped and ended early if students continue to dance or behave inappropriately.
